Introducción. Habitualmente, durante la manometría anorrectal, en lo correspondiente al reflejo rectoanal inhibitorio (RRAI) solo se pesquisa su presencia o ausencia. Estudios han reportado que su análisis detallado puede brindar datos de interés. Nuestra hipótesis es que la medición del RRAI puede dar información para reconocer causas orgánicas (médula anclada, lipoma, etc.) en pacientes en los que previamente se consideró como de causa funcional. Objetivos. Comparar la duración del reflejo rectoanal inhibitorio en la manometría anorrectal de pacientes con constipación funcional refractaria (CFR) y mielomeningocele (MMC). Población y métodos. Estudio observacional, transversal, analítico (2004-2019). Pacientes constipados crónicos con incontinencia fecal funcional y orgánica (mielomeningocele). Se les realizó manometría anorrectal con sistema de perfusión de agua y se midió la duración del RRAI con diferentes volúmenes (20, 40 y 60 cc). Grupo 1 (G1): 81 CFR. Grupo 2 (G2): 54 MMC. Se excluyeron pacientes con retraso madurativo, esfínter anal complaciente, agenesia sacra y aquellos no colaboradores. Resultados. Se incluyeron 135 sujetos (62 varones). La mediana de edad fue G1:9,57 años; G2: 9,63 años. Duración promedio G1 vs. G2 con 20 cc: 8,89 vs. 15,21 segundos; con 40 cc: 11.41 vs. 21,12 segundos; con 60 cc: 14,15 vs. 26,02 segundos. La diferencia de duración del RRAI entre ambos grupos con diferentes volúmenes fue estadísticamente significativa (p = 0,0001). Conclusión. La duración del RRAI aumenta a mayor volumen de insuflación del balón en ambas poblaciones. Pacientes con MMC tuvieron mayor duración del RRAI que aquellos con CFR. En los pacientes con RRAI prolongado, debe descartarse lesión medular.


Introduction. Usually, during anorectal manometry, only the presence or absence of rectoanal inhibitory reflex (RAIR) is investigated. Studies have reported that a detailed analysis may provide data of interest. Our hypothesis is that RAIR measurement may provide information to detect organic causes (tethered cord, lipoma, etc.) in patients in whom a functional cause had been previously considered. Objectives. To compare RAIR duration in anorectal manometry between patients with refractory functional constipation (RFC) and myelomeningocele (MMC). Population and methods. Observational, analytical, cross-sectional study (2004­2019). Patients with chronic constipation and functional and organic fecal incontinence (myelomeningocele). The anorectal manometry was performed with a water-perfused system, and the duration of RAIR was measured with different volumes (20, 40, and 60 cc). Group 1 (G1): 81 RFC. Group 2 (G2): 54 MMC. Patients with developmental delay, compliant anal sphincter, sacral agenesis and non-cooperative patients were excluded. Results. A total of 135 individuals were included (62 were male). Their median age was 9.57 years in G1 and 9.63 years in G2. Average duration in G1 versus G2 with 20 cc: 8.89 versus 15.21 seconds; 40 cc: 11.41 versus 21.12 seconds; 60 cc: 14.15 versus 26.02 seconds. The difference in RAIR duration with the varying volumes was statistically significant (p = 0.0001). Conclusion. RAIR duration was longer with increasing balloon inflation volumes in both populations. RAIR duration was longer in patients with MMC than in those with RFC. Spinal injury should be ruled out in patients with prolonged RAIR.

ABSTRACT

In recent years, colonic manometry has been gradually introduced into clinical practice. It helps clinicians to gain a better understanding of the physiology and pathophysiology of colonic contractile activity in healthy adults and patients with colonic dysfunction. More and more patterns of colonic motility are being discovered with the help of colonic manometry. However, the clinical significance of these findings still needs to be further investigated. This review enhances our understanding of colonic motility and the current state of development and application of colonic manometry, as well as the limitations, future directions and potential of the technique in assessing the impact of treatment on colonic motility patterns, by analyzing and summarizing the literature related to colonic manometry.

Introducción: La manometría esofágica de alta resolución es la prueba ideal para el diagnóstico de la acalasia y muestra la presencia de ondas terciarias o aperistalsis y el aumento de presión, con ausencia de relajación, del esfínter esofágico inferior. Objetivo: Evaluar la utilidad de la manometría esofágica de alta resolución en el diagnóstico y clasificación de la acalasia esofágica. Métodos: Se realizó una investigación descriptiva, trasversal, en el Centro Nacional de Cirugía de Mínimo Acceso entre octubre del 2018 y diciembre del 2019, en 46 pacientes con diagnóstico de acalasia esofágica. Se excluyeron aquellos con cirugía previa del esófago. Las variables incluidas fueron: edad, sexo, tiempo de evolución, síntomas y hallazgos manométricos. Para el análisis de las variables cuantitativas se emplearon medidas de tendencia central, media y de dispersión, la desviación estándar. Las frecuencias y proporciones fueron utilizadas para describir las variables cualitativas. Resultados: Predominó la acalasia tipo II, en el sexo femenino (57 por ciento). La disfagia fue el síntoma más frecuente (84, 76 y 100 por ciento en los tipos I, II y III, respectivamente). El 70 por ciento de los casos presentó más de un año de evolución de los síntomas. La media de la presión de relajación integrada estuvo por encima de 21 mmHg independientemente del tipo. No se encontraron pacientes con subtipos de la acalasia tipo III. Conclusiones: La manometría esofágica de alta resolución es útil para el diagnóstico y clasificación de la acalasia esofágica(AU)


Introduction: High-resolution esophageal manometry is the ideal test for the diagnosis of achalasia and shows the presence of tertiary waves or aperistalsis and increased pressure, in the absence of relaxation, of the lower esophageal sphincter. Objective: To assess the usefulness of high-resolution esophageal manometry in the diagnosis and classification of esophageal achalasia. Methods: A descriptive, cross-sectional investigation was carried out at the National Center for Minimal Access Surgery, between October 2018 and December 2019, in 46 patients with a diagnosis of esophageal achalasia. Those with previous esophageal surgery were excluded. The variables included were: age, sex, time of evolution, symptoms and manometric findings. For the analysis of the results, the percentage and measures of central tendency (arithmetic mean and standard deviation) were used. Results: Type II achalasia predominated in females (57 percent). Dysphagia was the most frequent symptom (84, 76, and 100 percent in types I, II, and III, respectively). 70 percent of the cases presented more than one year of evolution of the symptoms. Regarding the high-resolution manometry parameters, it was observed that regardless of the type, the mean integrated relaxation pressure was above 21 mmHg. No patients with type III achalasia subtypes were found. Conclusions: High-resolution esophageal manometry is useful for the diagnosis and classification of esophageal acalasia(AU)

Resumen Introducción: La hernia hiatal (HH) de tipo I por deslizamiento es el tipo más frecuente, siendo difícil de definir objetivamente, por lo que el principal foco de controversia es su diagnóstico. El objetivo del presente trabajo es reportar los resultados respecto de la precisión diagnóstica del estudio preoperatorio y confirmarlo con el diagnóstico laparoscópico de este tipo de HH. Materiales y Método: Estudio prospectivo descriptivo de serie que incluyen pacientes con síntomas típicos de enfermedad por reflujo gastroesofágico, los cuales se sometieron a estudio con esófago-gastro-duodenoscopía, estudio manométrico y radiológico de esófago, estómago y duodeno con bario. Se incluyen sólo los pacientes en los cuales la endoscopía revela la existencia de HH por deslizamiento ≪ 5 cm. Estos pacientes se sometieron a tratamiento quirúrgico confirmándose o no la existencia de HH al momento de la exploración laparoscópica. Resultados: El valor predictivo positivo y sensibilidad para manometría fue de un 51,2% y 70%, para la radiología 91,7% y 80,5% y para endoscopia 80,3% y 70,7% respectivamente. Conclusión: Para el diagnóstico confiable de HH antes del tratamiento, las tres investigaciones mencionadas deben ser obligatorias antes de la cirugía.


Introduction: Being type I hiatal hernia (HH) the most frequent, is difficult to define objectively and therefore, the main focus of controversy is the diagnosis. The aim of this paper is to report the results regarding the diagnostic accuracy of the preoperative study and to confirm it with the laparoscopic diagnosis of hiatal hernia. Materials and Method: This descriptive and prospective study includes patients with typical symptoms of gastroesophageal reflux disease who underwent esophageal-gastro-duodenoscopy, manometry and radiological study of esophagus with barium swallow. Only patients in whom endoscopy reveals the existence of HH by sliding ≪ 5 cm are included. These patients underwent surgical treatment confirming or not the existence of HH at the time of laparoscopic exploration. Results: The positive pre- dictive value and sensibility for manometry was 51.2% and 70%, for radiology 91.7% and 80.5%, and for endoscopy 85.3% and 70.7% respectively. Conclusion: For the reliable diagnosis of HH before treatment, the three mentioned investigations must be mandatory before the surgery.

RESUMEN: Introducción: La manometría esofágica de alta resolución se considera a nivel internacional como un método óptimo para el diagnóstico de los trastornos motores del esófago, de alta especificidad y sensibilidad. Dada la reciente introducción de la técnica en Cuba, se necesita demostrar su utilidad en nuestro país, donde no existe grado de conocimiento previo sobre cuáles son los parámetros de normalidad, así como los diagnósticos de alta resolución más frecuentes. Objetivo: Determinar la utilidad de la manometría esofágica de alta resolución como estudio diagnóstico para los trastornos motores del esófago en Cuba. Métodos: Se diseñó una investigación aplicada, de desarrollo, realizada en el Centro Nacional de Cirugía de Mínimo Acceso, en La Habana, entre junio de 2017 y junio de 2019, en pacientes a los que se les realizó una manometría esofágica. El universo de pacientes fue 611, a quienes se les registraron las variables en estudio. Resultados: De las 611 manometrías esofágicas, 356 correspondían a mujeres (58,3 por ciento) y 255 a hombres (42,7 por ciento), con un promedio de edad de 47, 44 años respectivamente. La motilidad inefectiva fue el trastorno motor más frecuente (32,73 por ciento). En los pacientes con Enfermedad por Reflujo Gastroesofágico, el principal daño de la barrera antirreflujo fue la presencia de relajaciones transitorias del esfínter esofágico inferior. Predominó la Acalasia tipo II (5,23 por ciento). En estos pacientes se registraron valores de Presión de relajación integrada (IRP) por encima de 21. Conclusiones: Se establecieron valores aplicables a pacientes cubanos, que permiten establecer el diagnóstico por alta resolución(AU)


ABSTRACT Introduction: High-resolution esophageal manometry is considered internationally as an optimal method for diagnosis of esophageal motor disorders. Given the recent introduction of the technique in Cuba, it is necessary to demonstrate its usefulness in our country where there is no prior knowledge about the most frequent parameters of normality and high resolution diagnoses. Objective: To determine the usefulness of high-resolution esophageal manometry as a method for the diagnosis of esophageal motor disorders in Cuba. Material and methods: An applied developmental research was designed and carried out at the National Center for Minimal Access Surgery in Havana between June 2017 and June 2019. The study included patients in whom esophageal manometry was performed. The universe consisted of 611 patients and the variables used in the study were recorded. Results: Of the 611 patients who underwent esophageal manometry, 356 were women (58.26 percent) and 255 were men (42.73 percent), with an average age of 47 and 44 years, respectively. Ineffective motility was the most frequent motor disorder (32,73 percent). In patients with Gastroesophageal Reflux Disease, the main damage of the antireflux barrier was the presence of transient lower esophageal sphincter relaxations. Type II achalasia predominated (5,23 percent). In these patients, integrated relaxation pressure (IRP) values above 21 were recorded. Conclusions: Values applicable to Cuban patients that allow to establish a high-resolution diagnosis were established(AU)

Abstract Background Changes in arterial compliance are among the first changes detectable in hypertensive syndrome. Methods with good reproducibility as compared with the gold standard for identifying such changes are desirable in clinical practice. Objectives To compare central pressure measurements and arterial stiffness obtained by two non-invasive methods (tonometry and oscillometry). Methods This was a descriptive, cross-sectional study with a convenience sample of patients with borderline hypertension or stage 1 hypertension. Peripheral and central blood pressure measurements were obtained by tonometry (SphygmoCor®), considered the gold standard, and oscillometry (Mobil O´graph®). Comparisons of results were made by unpaired t-test, and p values < 0.05 were considered statistically significant. Results No difference was found in central pressure measurements obtained by SphygmoCor® (117 x 80.1 mmHg) compared with Mobil O'graph (112 x 81.4 mmHg). Mean augmentation index (AIx) was 26.1% and 21.3%, and mean pulse pressure (PP) amplification 10.7 mmHg and 10.0 mmHg by Sphygmocor® and Mobil O´graph®, respectively (p > 0.05). Mean pressure wave velocity (PWV), 8.4 m/s vs. 7.4 m/s (p = 0.013) and mean central pulse pressure, 37.7 mmmHg and 30.9 mmHg (p = 0.013) were significantly higher by SphygmoCor® than Mobil O´graph®. Conclusion Values of central systolic blood pressure, AIx and pulse pressure amplification obtained by oscillometry were not statistically different compared with tonometry; values of PWV and cPP, however, were underestimated by oscillometry. Abstract Background: Lichen planus is a chronic inflammatory mucocutaneous disease. Recent studies have suggested that it is associated with an increased risk of cardiovascular comorbidities. Objective: The purpose of this study was to assess and compare arterial stiffness and cardiovascular hemodynamics in patients with lichen planus and a healthy control group. Methods: Fifty-five patients with lichen planus and 42 healthy controls were enrolled. All patients underwent echocardiographic examination, and arterial stiffness was measured using applanation tonometry. Results: No statistically significant difference was determined between the patient and control groups in terms of arterial stiffness, but stiffness was markedly higher in patients with erosive lichen planus compared to the control group and other patients (p = 0.006, and p = 0.023, respectively). Moderate positive correlation was determined between duration of disease and arterial stiffness. Impairment of systolic and diastolic functions was also determined in patients with lichen planus compared to the control group (p < 0.001, and p = 0.005, respectively). Study limitations: Relatively low number of patients. Conclusion: The positive correlation observed between duration of disease and arterial stiffness in patients with lichen planus suggests that these patients should be followed-up in terms of cardiovascular risk in the presence of resistant and long-term disease, particularly in case of erosive lichen planus.

SUMMARY INTRODUCTION Systemic sclerosis (SSC) is an autoimmune disorder that affects several organs of unknown etiology, characterized by vascular damage and fibrosis of the skin and organs. Among the organs involved are the esophagus and the lung. OBJECTIVES To relate the profile of changes in esophageal electromanometry (EM), the profile of skin involvement, interstitial pneumopathy (ILD), and esophageal symptoms in SSC patients. METHODS This is an observational, cross-sectional study carried out at the SSC outpatient clinic of the Hospital de Clínicas of the Federal University of Uberlândia. After approval by the Ethics Committee and signed the terms of consent, 50 patients were initially enrolled, from 04/12/2014 to 06/25/2015. They were submitted to the usual investigations according to the clinical picture. The statistical analysis was descriptive in percentage, means, and standard deviation. The Chi-square test was used to evaluate the relationship between EM, high-resolution tomography, and esophageal symptoms. RESULTS 91.9% of the patients had some manometric alterations. 37.8% had involvement of the esophageal body and lower esophageal sphincter. 37.8% had ILD. 24.3% presented the diffuse form of SSC. No association was found between manometric changes and clinical manifestations (cutaneous, pulmonary, and gastrointestinal symptoms). CONCLUSION The present study confirms that esophageal motility alterations detected by EM are frequent in SSC patients, but may not be related to cutaneous extension involvement, the presence of ILD, or the gastrointestinal complaints of patients.


RESUMO INTRODUÇÃO A esclerose sistêmica (ES) é uma doença autoimune que afeta vários órgãos de etiologia desconhecida, caracterizada por dano vascular e fibrose da pele e órgãos. Entre os órgãos envolvidos estão o esôfago e o pulmão. OBJETIVOS Relacionar o perfil das alterações na eletromanometria (ME), o perfil de acometimento da pele, a pneumopatia intersticial (PI) e os sintomas esofágicos em pacientes com ES. MÉTODO Trata-se de um estudo observacional, transversal, realizado no ambulatório de SSC do Hospital das Clínicas da Universidade Federal de Uberlândia. Após aprovação pelo Comitê de Ética e assinatura dos termos de consentimento, 50 pacientes foram inicialmente convidados, de 04/12/2014 a 25/06/2015. Eles foram submetidos às investigações usuais de acordo com o quadro clínico. A análise estatística foi descritiva em porcentagem, média e desvio padrão. O teste Qui-quadrado foi utilizado para avaliar a relação entre ME, tomografia de alta resolução e sintomas esofágicos. RESULTADOS 91,9% dos pacientes apresentaram alterações manométricas. 37,8% tinham envolvimento do corpo esofágico e do esfíncter esofágico inferior. 37,8% tinham IP. 24,3% apresentaram a forma difusa da ES. Não há associação entre alterações manométricas e manifestações clínicas (sintomas cutâneos, pulmonares e gastrointestinais). CONCLUSÃO O presente estudo confirma que as alterações da motilidade esofágica detectadas pela EM são frequentes em pacientes com SSC, mas podem não estar relacionadas ao envolvimento cutâneo, à de DPI ou às queixas gastrointestinais dos pacientes.

ABSTRACT Background: High-resolution manometry is more costly but clinically superior to conventional manometry. Water-perfused systems may decrease costs, but it is unclear if they are as reliable as solid-state systems, and reference values are interchangeable. Aim: To validate normal values for a new water-perfusion high-resolution manometry system. Methods: Normative values for a 24-sensors water perfused high-resolution manometry system were validated by studying 225 individuals who underwent high resolution manometry for clinical complaints. Patients were divided in four groups: group 1 - gastroesophageal reflux disease; group 2 - achalasia; group 3 - systemic diseases with possible esophageal manifestation; and group 4 - dysphagia. Results: In group 1, a hypotonic lower esophageal sphincter was found in 49% of individuals with positive 24 h pH monitoring, and in 28% in pH-negative individuals. In groups 2 and 3, aperistalsis was found in all individuals. In group 4, only one patient (14%) had normal high-resolution manometry. Conclusions: The normal values determined for this low-cost water-perfused HRM system with unique peristaltic pump and helicoidal sensor distribution are discriminatory of most abnormalities of esophageal motility seen in clinical practice.


RESUMO Racional: A manometria de alta resolução é mais custosa, porém clinicamente superior à manometria convencional. Sistemas por perfusão de água podem ter custo diminuído, mas não é certo se são tão eficazes quanto aos sistemas de estado sólido e se os valores de referência são intercambiáveis. Objetivo: Este estudo visa validar valores de normalidade para um novo sistema por perfusão de água. Método: Valores de normalidade para um sistema de manometria de alta resolução de 24 sensores por perfusão de água foram validados estudando 225 indivíduos submetidos à manometria de alta resolução por queixas clínicas. Pacientes foram divididos em quatro grupos: grupo 1 - doença do refluxo gastroesofágico; grupo 2 - acalasia; grupo 3 - doenças sistêmicas com possível doenças sistêmicas com comprometimento esofágico; e grupo 4 - pacientes com disfagia. Resultado: No grupo 1, esfíncter esofagiano inferior hipotônico foi encontrado em 49% dos indivíduos com pHmetria positiva e 28% daqueles com pHmetria negativa. Nos grupos 2 e 3, aperistalse foi encontrada em todos indivíduos. No grupo 4, somente um paciente (14%) tinha manometria normal. Conclusão: Os valores de normalidade definidos para este sistema de manometria de alta resolução por perfusão de água são discriminatórios da maioria das anormalidades da motilidade esofágica vistas na prática clínica.

Introducción: La superación profesional, contribuye a la educación permanente del profesional para lograr su actualización, dirigido a la superación profesional de gastroenterólogos especialistas y residentes del Servicio de Gastroenterología Hospital Clínico Quirúrgico "Hermanos Ameijeiras" sobre la técnica de Manometría Esofágica. Objetivos: Diseñar un curso de superación para mejorar el conocimiento e interpretación de la Manometría Esofágica. Métodos: Se realizó un estudio de enfoque mixto, educativo, de desarrollo que culmina con el diseño del curso en Manometría Esofágica dirigido a residentes y especialistas en gastroenterología del Servicio de Gastroenterología del Hospital Hermanos Ameijeiras, para lo cual se trabajó con los diez residentes y siete especialistas del servicio, además de tres profesores consultantes de gran experiencia, considerados expertos. Se utilizó la encuesta para residentes y especialistas y entrevista en profundidad con los profesores consultantes. El método estadístico permitió presentación y análisis de los datos cuantitativos y el análisis de contenido para las entrevistas a los expertos. Se consideraron los aspectos éticos requeridos para la investigación, de la institución y los involucrados en el estudio. Resultados: El 100 por ciento de participantes, carecen de conocimientos y habilidades en el uso de la Manometría Esofágica. Se diseñó un curso básico- teórico que contribuirá a prepararlos para hacer diagnóstico, pronóstico y tratamiento en la Enfermedad por Reflujo Gastroesofágico, la Acalasia y otros trastornos motores del esófago. Conclusiones: Las necesidades de aprendizaje, de residentes y especialistas, justificaron el diseño del curso, contribuirá a superar a gastroenterólogos en el manejo de las afecciones mencionadas(AU)


Introduction: Professional improvement contributes to permanent professional education to achieve updated levels, aimed at the professional improvement of gastroenterologists, both residents and specialists, of the Gastroenterology Service of Hermanos Ameijeiras Clinical-Surgical Hospital regarding the technique of esophageal manometry. Objectives: To design an updating course for improving the knowledge and interpretation of esophageal manometry. Methods: A mixed, educational, and developmental study was carried out, which culminated in the design of the course in Esophageal Manometry for residents and specialists in gastroenterology of the Gastroenterology Service of Hermanos Ameijeiras Hospital, for which work was done with the ten residents and seven specialists of the service, as well as three experienced consultant professors, considered experts. We used the survey for residents and specialists and the in-depth interview with the consulting professors. The statistical method allowed the presentation and analysis of the quantitative data and the content analysis for the interviews with the experts. The ethical aspects required for the investigation were considered, from the institution and those involved in the study. Results: 100 percent of participants lacked knowledge and skills in the use of esophageal manometry. A basic theoretical course was designed to help prepare them for diagnosis, prognosis and treatment regarding gastroesophageal reflux disease, achalasia, and other motor disorders of the esophagus. Conclusions: The learning needs of residents and specialists justified the course design, and will help overcome gastroenterologists in the management of the aforementioned conditions(AU)

RESUMEN Objetivo La asociación entre diabetes mellitus tipo 2 y glaucoma primario de ángulo abierto aún no está claramente establecida. Este estudio tiene como objetivo identificar la frecuencia entre ambas patologías en pacientes del Hospital Teodoro Maldonado Carbo durante el período 2010-2014 Métodos Se realizó un estudio retrospectivo, de corte transversal, de proporción, en una muestra de 283 pacientes mayores de 30 años cuyo diagnóstico fue de GPAA (Glaucoma primario de ángulo abierto) confirmado mediante estudio de tonometría, gonioscopia, campimetría y ausencia de causas secundarias de aumento de PIO. Se utilizaron criterios de la ADA/OMS para establecer el diagnóstico de DM2. Resultados La frecuencia de pacientes con diabetes en pacientes con diagnóstico de GPAA, que fueron atendidos en consultorios de Emergencia, Endocrinología y Oftalmología del Hospital Regional Teodoro Maldonado Carbo, durante el período 2010-2014, fue de 26,15% (74 de 283 pacientes), con importantes diferencias entre sexo y lugar de residencia. Conclusiones: Se demostró la existencia de una alta proporción entre DM2 y GPAA mediante razón de proporción.


ABSTRACT Objective The association between diabetes mellitus type 2 (DM2) and Primary Open- Angle Glaucoma (POAG) is not clearly established yet. This study has the objective to identify the frequency of both diseases in patients of Teodoro Maldonado Carbo Hospital during the period 2010-2014. Methods A retrospective, cross-sectional frequency was conducted on a sample of 283 patients over age 30 whose diagnosis was confirmed by examination of POAG tonometry, gonioscopy, perimetry and absence of secondary causes of increased intraocular pressure. ADA/OMS criteria were used to establish the diagnosis of DM2. Results The frequency of patients with diabetes diagnosed with POAG patients who were treated at the services of Emergency, Endocrinology and Ophthalmology of the Regional Hospital Teodoro Maldonado Carbo, during the period 2010-2014, was 26.15% (74 of 283 patients) with significant differences between sex and place of residence. Conclusions The existence of a high proportion between DM2 and POAG was demonstrated by prevalence ratio.

Resumen Introducción: La técnica de elección para la reconstrucción del tránsito intestinal después de una gastrectomía total es la esófago-yeyuno anastomosis en Y de Roux, la cual evita el reflujo de jugo digestivo alcalino. El objetivo del presente estudio es el de analizar la motilidad del esófago y del asa de yeyuno en pacientes operados con gastrectomía total por cáncer gástrico. Pacientes y método: Se estudió prospectivamente una muestra de 17 pacientes intervenidos mediante gastrectomía total 5 años atrás. En todos ellos se realizaron manometrías esofágicas de alta resolución incluyendo los primeros 7 cm del asa de yeyuno. Resultados: Los resultados de la manometría demostraron motilidad esofágica normal a excepción de la presión máxima intrabolo, la cual se encontraba aumentada en todos los pacientes. La motilidad del asa de yeyuno fue desordenada e inefectiva. Conclusiones: La motilidad del cuerpo esofágico es normal 5 años después de la gastrectomía total. Sin embargo, la presión intrabolo máxima se encuentra aumentada; la causa más probable de este hecho estaría constituida por el asa de yeyuno, que ejerce resistencia al paso del bolo debido a que presenta una actividad motora desordenada y poco propulsiva. El impacto clínico de este hecho es, sin embargo, poco importante.


Abstract Introduction: The most preferred technique to reconstruct the intestinal transit after total gastrectomy for gastric cancer is the Roux-en-Y esophagus-gastrostomy which prevents the reflux of alkaline intestinal juice. The purpose of this study was to analyze the esophageal motility and the motility of the jejunal loop in patients subjected to total gastrectomy. Patients and method: A prospective sample of 17 total gastrectomy patients operated on 5 years before was studied using high resolution esophageal manometry including the first 7 cm of the jejunal loop. Results: Manometry results showed normal esophageal body motility with the exception of the maximum intrabolus pressure that was elevated in all patients. The jejunal loop motility was disordered and ineffective. Conclusions: Esophageal body motility was normal 5 years after the surgical procedure. However, maximum intrabolus pressure was elevated and the most plausible reason would be the jejunal loop that exerts resistance to the bolus passage due to its ineffective motility. However, this fact does not have a significant clinical impact.

ABSTRACT BACKGROUND Esophageal motility has been described in the literature as having differences between men and women. Most of these investigations use the water perfusion method for esophageal manometry. In this investigation the esophageal motility of men and women was compared with high-resolution manometry of the esophagus. OBJECTIVE To compare the esophageal motility of men and women with the high-resolution manometry method for esophageal manometry, performed in the sitting position. The hypothesis was that men and women have differences in esophageal motility. METHODS High-resolution manometry was performed in normal volunteers, 10 men [mean age: 37.5 (8.1) years] and 12 women [mean age: 38.7 (7.5) years], in the sitting position and with 10 swallows of a 5 mL bolus of saline, with an interval of at least 30 seconds between consecutive swallows. We evaluated the integrated relaxation pressure of the lower esophageal sphincter, contraction front velocity, distal contraction integral, distal latency, proximal contraction extension, proximal contraction duration >30 mmHg, proximal contraction duration, proximal contraction integral and maximal upper esophageal sphincter pressure. RESULTS There was no significant difference between men and women in the variables measured. CONCLUSION There was no difference in esophageal motility of men and women evaluated by the high resolution manometry method, in the sitting position with swallows of a liquid bolus.


RESUMO CONTEXTO É descrita a existência de diferenças na motilidade de esôfago entre homens e mulheres. A maioria destes trabalhos utilizaram o método de perfusão continua com água para a manometria esofágica. Nesta investigação foi comparada a motilidade do esôfago de homens e mulheres com o método de manometria de alta resolução, realizada na posição sentada e com deglutição de bolo líquido. OBJETIVO Comparar a motilidade do esôfago em homens e mulheres, na posição sentada, com o método de manometria de alta resolução. A hipótese é que homens e mulheres têm diferenças na motilidade do esôfago. MÉTODOS Manometria de alta resolução foi realizada em voluntários saudáveis, 10 homens [média de idade: 37,7 (8,1) anos] e 12 mulheres [média de idade: 38,7 (7,5) anos], na posição sentada e com 10 deglutições de 5 mL de solução salina. Foram avaliadas a pressão integrada de relaxamento do esfíncter inferior do esôfago, velocidade da contração peristáltica, integral da contração distal, latência distal, extensão da contração proximal, duração da contração proximal >30 mmHg, duração da contração proximal, integral da contração proximal, pressão máxima do esfíncter superior do esôfago. RESULTADOS Não houve diferença significativa entre homens e mulheres nas variáveis medidas. CONCLUSÃO Não há diferença entre homens e mulheres na motilidade do esôfago avaliada pelo método de manometria de alta resolução, na posição sentada e com deglutição de um bolo líquido.

ABSTRACT Introduction: High resolution manometry is the current technology used to the study of esophageal motility and is replacing conventional manometry in important centers for esophageal motility with parameters used on esophageal motility, following the Chicago Classification. This classification unifies high resolution manometry interpretation and classifies esophageal disorders. Objective: This review shows, in a pictorial presentation, the new parameters established by the Chicago Classification, version 3.0, aimed to allow an easy comprehension and interpretation of high resolution manometry. Methods: Esophageal manometries performed by the authors were reviewed to select illustrative tracings representing Chicago Classification parameters. Results: The parameters are: Esophagogastric Morphology, that classifies this junction according to its physiology and anatomy; Integrated Relaxation Pressure, that measures the lower esophageal sphincter relaxation; Distal Contractile Integral, that evaluates the contraction vigor of each wave; and, Distal Latency, that measures the peristalsis velocity from the beginning of the swallow to the epiphrenic ampulla. Conclusion: Clinical applications of these new concepts is still under evaluation.


RESUMO A manometria de alta resolução é, atualmente, a tecnologia mais moderna para o estudo da motilidade esofágica e vem substituindo a manometria convencional nos grandes centros de pesquisa com parâmetros que seguem a Classificação de Chicago, que busca unificar as interpretações gráficas da manometria de alta resolução e, dessa maneira, categorizar os diversos distúrbios esofágicos. Objetivo: Mostrar, de forma pictórica, os novos parâmetros compilados na versão 3.0 da Classificação de Chicago, buscando facilitar a compreensão e interpretação da manometria de alta resolução. Métodos: Foram revistas as manometrias da casuística dos autores e selecionados os traçados representativos dos parâmetros da Classificação de Chicago. Resultados: Entre os parâmetros apresentados foram considerados a Morfologia da Transição Gastroesofágica, que classifica o segmento de acordo com sua fisiologia e anatomia; a Integral da Pressão de Relaxamento, que mede o relaxamento do esfíncter esofagiano inferior; a Integral Contrátil Distal, que avalia o vigor contrátil da onda peristáltica; e, a Latência Distal, que mede o tempo da peristalse, desde o início da deglutição até a ampola epifrênica. Conclusão: A aplicabilidade clínica desses novos conceitos ainda está sendo estudada.

RESUMO Objetivo: Avaliar a utilidade e o valor prognóstico da tonometria arterial periférica - hiperemia reativa em pacientes com sepse, e investigar a associação dos resultados deste exame com os níveis séricos de algumas moléculas inflamatórias. Métodos: Estudo prospectivo, realizado em uma unidade de terapia intensiva para pacientes adultos com 18 leitos. Os critérios de exclusão foram imunossupressão grave ou tratamento com antibióticos iniciado mais de 48 horas antes da avaliação. Aplicamos o exame de tonometria arterial periférica - hiperemia reativa quando da inclusão (dia 1) e no dia 3. Avaliamos os níveis de interleucina 6, interleucina 10, proteínas do grupo 1 de mobilidade alta e de ST2 solúvel no sangue obtido quando da inclusão. Resultados: Dos 79 pacientes incluídos, 17 (21,6%) tiveram os sinais da tonometria arterial periférica - hiperemia reativa considerados tecnicamente não confiáveis, tendo sido excluídos do estudo. Assim, incluímos na análise final 62 pacientes, que foram submetidos a 95 exames de tonometria arterial periférica - hiperemia reativa dentro das primeiras 48 horas após sua inclusão. A média de idade foi de 51,5 (DP: 18,9), e 49 (62%) dos pacientes eram do sexo masculino. Os índices de hiperemia reativa dos dias 1 e 3 não se associaram com necessidade de vasopressores, SOFA, APACHE II ou mortalidade aos 28 dias. Dentre os pacientes que morreram, em comparação aos sobreviventes, houve aumento significante nos índices de hiperemia reativa no dia 3 em comparação ao dia 1 (p = 0,0045). Ocorreu fraca correlação negativa entre o índice obtido por tonometria arterial periférica - hiperemia reativa no dia 1 e os níveis de proteínas do grupo 1 de mobilidade alta (r = -0,287). Conclusão: Dificuldades técnicas e falta de associações claras dos resultados do exame com a gravidade clínica e com o desfecho foram fortes limitantes da utilidade do exame de tonometria arterial periférica - hiperemia reativa em pacientes sépticos admitidos à unidade de terapia intensiva.


ABSTRACT Objective: To evaluate the usefulness and prognostic value of reactive hyperemia - peripheral arterial tonometry in patients with sepsis. Moreover, we investigated the association of reactive hyperemia - peripheral arterial tonometry results with serum levels of certain inflammatory molecules. Methods: Prospective study, conducted in an 18-bed mixed intensive care unit for adults. The exclusion criteria included severe immunosuppression or antibiotic therapy initiated more than 48 hours before assessment. We measured the reactive hyperemia - peripheral arterial tonometry on inclusion (day 1) and on day 3. Interleukin-6, interleukin-10, high-mobility group box 1 protein and soluble ST2 levels were measured in the blood obtained upon inclusion. Results: Seventeen of the 79 patients (21.6%) enrolled were determined to have reactive hyperemia - peripheral arterial tonometry signals considered technically unreliable and were excluded from the study. Thus, 62 patients were included in the final analysis, and they underwent a total of 95 reactive hyperemia - peripheral arterial tonometry exams within the first 48 hours after inclusion. The mean age was 51.5 (SD: 18.9), and 49 (62%) of the patients were male. Reactive hyperemia indexes from days 1 and 3 were not associated with vasopressor need, Sequential Organ Failure Assessment score, Acute Physiology and Chronic Health Evaluation II score, or 28-day mortality. Among the patients who died, compared with survivors, there was a significant increase in the day 3 reactive hyperemia index compared with day 1 (p = 0.045). There was a weak negative correlation between the day 1 reactive hyperemia - peripheral arterial tonometry index and the levels of high-mobility group box 1 protein (r = -0.287). Conclusion: Technical difficulties and the lack of clear associations between the exam results and clinical severity or outcomes strongly limits the utility of reactive hyperemia - peripheral arterial tonometry in septic patients admitted to the intensive care unit.

Summary Introduction: Digital palpation and manometry are methods that can provide information regarding maximum voluntary contraction (MVC) and endurance of the pelvic floor muscles (PFM), and a strong correlation between these variables can be expected. Objective: To investigate the correlation between MVC and endurance, measured by digital palpation and manometry. Method: Forty-two women, with mean age of 58.1 years (±10.2), and predominant symptoms of stress urinary incontinence (SUI), were included. Examination was firstly conducted by digital palpation and subsequently using a Peritron manometer. MVC was measured using a 0-5 score, based on the Oxford Grading Scale. Endurance was assessed based on the PERFECT scheme. Results: We found a significant positive correlation between the MVC measured by digital palpation and the peak manometric pressure (r=0.579, p<0.001), and between the measurements of the endurance by Peritron manometer and the PERFECT assessment scheme (r=0.559, P<0.001). Conclusion: Our results revealed a positive and significant correlation between the capacity and maintenance of PFM contraction using digital and manometer evaluations in women with predominant symptoms of SUI.


Resumo Introdução: a palpação digital e a manometria são métodos capazes de fornecer informações sobre contração voluntária máxima (CVM) e endurance da musculatura do assoalho pélvico (MAP), e pode-se esperar uma forte correlação entre essas variáveis. Objetivo: investigar a correlação entre CVM e endurance, avaliados por palpação digital e manometria. Método: incluíram-se 42 mulheres, com idade média de 58,1 anos (±10,2) e sintomas predominantes de incontinência urinária de esforço (IUE). Realizou-se primeiramente o exame digital, seguido pela manometria (Peritron®). Mensuraram-se a CVM de acordo com a escala de Oxford (0-5 pontos) e o endurance pelo esquema PERFECT. Resultados: encontrou-se correlação positiva entre CVM mensurada por palpação digital e pressão manométrica de pico (r=0,579; p<0,001), e entre as medições do endurance avaliado pelo Peritron e o esquema PERFECT (r=0,559; p<0,001). Conclusão: os resultados revelaram correlação positiva e significativa entre a capacidade e a manutenção de contração dos MAP por meio das avaliações digital e manométrica em mulheres com IUE.

INTRODUCCIÓN: el estreñimiento es un síntoma o afección que puede comprometer la calidad de vida de los pacientes. Su prevalencia global varía entre 2 % y 28 %. OBJETIVO: evaluar la concordancia entre los resultados de los exámenes imagenológicos y la manometría anorrectal, en pacientes con sospecha de estreñimiento funcional. MÉTODOS: la muestra estuvo conformada por 37 pacientes, a los que se les realizó exámenes radiológicos de cecocolografía secuencial (o colon por ingestión), tiempo de tránsito colónico con marcadores radiopacos, defecografía y examen manométrico. Se calculó el índice de Kappa para determinar la concordancia. RESULTADOS: 62,2 % de los enfermos fueron del sexo femenino, la edad predominante estuvo entre 41 y 60 años (43,2 %), predominó el ciego móvil (54,2 %) como causa de estreñimiento. Se hicieron 14 estudios de tiempo de tránsito colónico con marcadores radiopacos, de ellos 28,6 % presentó un tiempo de tránsito lento. Se realizó defecografía a 4 pacientes y 3 presentaron disfunción del suelo pelviano. Se constató que 94,6 % de las pruebas imagenológicas y 78,4 % de las manometrías resultaron positivas (Kappa p= 0,006). CONCLUSIÓN: las técnicas imagenológicas que se investigaron son apropiadas para diagnosticar el estreñimiento funcional y la concordancia de dichas técnicas con la manometría anorrectal es adecuada.


INTRODUCTION: constipation is a symptom or condition that can compromise the quality of patient's life. Its global prevalence varies between 2 % and 28 %. OBJECTIVE: evaluate the correlation between the results of imaging tests and anorectal manometry in patients with suspected functional constipation. METHODS: the sample consisted of 37 patients, who underwent radiological examinations of sequential cecocolografía (colon or swallowed), colonic transit time with radiopaque markers, defecography and manometric examination. Kappa index was calculated to determine the correlation. RESULTS: 62.2 % of patients were female, the predominant age was between 41 and 60 years (43.2 %), mobile blind predominated (54.2 %) as a cause of constipation. 14 studies of colonic transit time with radiopaque markers, were made, 28.6 % of them presented a slow transit time. 4 patients underwent defecography and 3 had pelvic floor dysfunction. It was found that 94.6 % of imaging tests and 78.4 % of manometries were positive (Kappa p= 0.006). CONCLUSIONS: imaging techniques used in this study are suitable for diagnosing functional constipation and consistency of these techniques is suitable anorectal manometry.

ABSTRACT INTRODUCTION: The use of handheld devices that assess peripheral arterial tonometry has emerged as an auxiliary method for assessment and diagnosis of obstructive sleep apnea syndrome. OBJECTIVE: To evaluate the accuracy of peripheral arterial tonometry in the diagnosis of obstructive sleep apnea. METHODS: Contemporary cohort cross-sectional study. Thirty patients with suspected obstructive sleep apnea underwent peripheral arterial tonometry and assisted nocturnal polysomnography concomitantly. RESULTS: The mean apnea/hypopnea index by peripheral arterial tonometry was significantly higher than that by polysomnography (p < 0.001), but the values of both sleep studies were significantly correlated (r = 0.762). There was a high correlation between variables: minimum oxygen saturation (r = 0.842,p < 0.001), oxygen saturation < 90% (r = 0.799, p < 0.001), and mean heart rate (r = 0.951, p < 0.001). Sensitivity and specificity were 60% and 96.2% (AUC: 0.727;p = 0.113), respectively, when at a threshold value of 5 events/h. In severe cases (≥30 events/h), the result was a sensitivity of 77.8% and a specificity of 86.4% (AUC: 0.846, p = 0.003). CONCLUSION: Peripheral arterial tonometry is a useful portable device for the diagnosis of obstructive sleep apnea; its accuracy is higher in moderate and severe cases.


RESUMO Introdução: A utilização de dispositivos portáteis, que avaliam a tonometria arterial periférica, surge como método adjuvante para avaliação e diagnóstico da síndrome da apneia obstrutiva do sono. Objetivo: Avaliar a acurácia da tonometria arterial periférica no diagnóstico da apneia obstrutiva do sono. Método: Estudo de coorte contemporânea com corte transversal. Trinta pacientes com suspeita de apneia obstrutiva do sono foram submetidos a tonometria arterial periférica e a polissonografia noturna assistida simultaneamente. Resultados: A média do índice de apneia/hipopneia pela tonometria arterial periféricafoi significativamente maior do que a da polissonografia (p < 0,001), porém os valores de ambos os estudos do sono foram significativamente correlacionados (r = 0,762). Houve alta correlação entre as variáveis: saturação mínima de oxigênio (r = 0,842, p < 0,001), saturação de oxigênio < 90% (r = 0,799, p < 0,001) e média de frequência cardíaca (r = 0,951, p < 0,001). A sensibilidade e especificidade foram 96,2% e 60% (AUC: 0,727, p = 0,113), respectivamente, quando limiar de 5 eventos/hora. Nos casos graves (≥ 30 eventos/hora), o resultado foi uma sensibilidade de 77,8% e uma especificidade de 86,4% (AUC: 0,846, p = 0,003). Conclusão: A tonometria arterial periférica é um dispositivo portátil útil no diagnóstico da apneia obstrutiva do sono e sua acurácia é maior nos casos moderados e graves.
